IBIS Breast Cancer Risk Evaluation Tool, which is based on the Tyrer-Cuzick risk model.
Instructions: Click on the Software Downloads tab and click on the icon in the Current Version section to download the program. Since the download is a Zip file, you'll need to uncompress it first with any freely available unzip program and then run the program. Only Windows and Pocket PC versions are available.
