To schedule an appointment, please call one of the three phone numbers in the left column. All services require an appointment and a script from your doctor.

We understand there may be times when you miss an appointment due to emergencies or obligations to work or family. However, we urge you to call 48-hours prior to canceling your appointment. If you no show for an appointment without 48-hour notice, you will be charged a $50 fee. If you no show for two consecutive appointments or no show for a total of three appointments, you may be discharged from care.
